In case anyone wanted the excruciating (public) details.

My last seven years in the AF I was the Safety Manager for my wing.
I was involved in countless Class C mishaps like FOD and bird strikes. Because of the costs assigned to A/C components FOD or bird strike damage is reportable as a misahap and requires a mishap report. I had three Class B’s, 1 FOD, one structural (unintended canopy jettison) and one where the BDU-33 spotting charge fired in the bay puncturing a 3″ fuel line and one Class A, non aircraft.
This investigation and mishap report will be used at the Safety Center at Kirtland as an example of what a Class A report should look like. All the excruciating details.
